PHYSICALS & CONCUSSION BASELINE TESTING

Sports Physicals for the 2011-2012 School Year
Our sports physicals will be held on Tuesday, June 14, 2011 at the stadium. PLEASE  CHECK DISTRICT WEBSITE for details including access to the Physical Packet and IMPACT testing information.  
 
The PRIHA require all players to have a physical and a baseline for concussion testing. Physicals are provided by the school for grades 7-12, grades 2-6 will have to be from a private physician. Impact testing (concussion) is on the website, the PRIHA is reccommending that all players have a baseline. 

Sports physicals will be held on Tuesday, June 14, 2011 at the stadium.  Parking will be at the middle school only.  Check website for the Physical Packet.  Please BRING your packet with you if you attend the physical on June 14.   Forms must be completed before you arrive at your approximate scheduled time listed on the form. 

Private physicals must be dated after June 1 and MUST be on the sports physical form to be accepted.  The packet is 9 pages and ALL pages must be turned in together.  We will NOT accept a one page form from the doctor.

If you get a physical at your own physician over the summer you will need to mail that to Pine-Richland High School, 700 Warrendale Road, Gibsonia, PA  15044, Attn: Athletics or drop it off in a sealed envelope at the middle school in a box marked sports physicals.

Fall Sports Physicals must be turned in by 8/8/11.
*physical forms for grades 2-6 are turned into the PRIHA at the latest by the first practice, all physical forms for grades 7-12 must be turned into the athletic office by 8/8/11*

The packet includes IMPACT testing information which lists testing times.  Do NOT CALL THE ATHLETIC OFFICE about IMPACT TESTING.  Call the trainers directly at 724-625-4444 Ext. 6825.  They will pick up their messages throughout the summer and will return your calls when they can.   Also, note that the IMPACT testing will be held at the Middle School.

*Inline physicals are scheduled between 10:45 and 11 am  on Tuesday June 14 for grades 7-12 at the High School Stadium *
Also wanted to let you know Wendy from UPMC South called me back with info about Impact baseline test.
They do not offer a discount group rate but only charge $25.00 for Baseline test.
Each participant will receive or be emailed a copy of the results.
She stated the athletes can schedule their own appointments it does not have to be a group schedule. As of right now, Baseline test can be scheduled at the UPMC South Campus. The are also going to be setting up off campus testing at the CCAC campuses sometime this summer.
